The Role
We are seeking a compassionate and dedicated Medical Social Worker (MSW) to provide home-based social work services to patients in the Longmont, Boulder, or Denver Metro area (30-mile radius). In this role, you will help patients and their families navigate healthcare challenges, connect them with community resources, and provide emotional support to improve their overall well-being.

- Conducting assessments of patients' psychosocial status and developing individualized care plans
- Providing counseling and emotional support to patients and families facing chronic illness or end-of-life care
Assisting with financial and resource planning, including insurance navigation and accessing community services
- Collaborating with the interdisciplinary team to ensure holistic, patient-centered care
- Advocating for patients’ rights and coordinating services to enhance quality of life
- Educating patients and families about available resources, coping strategies, and care options

Requirements
- What Makes You a Great Fit
- Master’s degree in Social Work (MSW) from an accredited program
- Valid Colorado Social Worker license
- Experience in home health, hospice, or medical social work preferred
- Strong knowledge of community resources, financial assistance programs, and healthcare systems
- Exceptional communication, problem-solving, and advocacy skills
- Licensed driver with current auto insurance and a good driving record
