Job Description

Job Overview

Fairview Range is hiring a Social Worker (BSW) for our Care Transitions Department.

As the Social Worker you will work under the direction of the director/supervisor the social worker provides discharge planning tools coordinating resources, and seamless transitions of care to families, individuals and groups. The social Worker complies with required standards for medical/legal responsibilities and serves as a resource to other allied and administrative staff within the Section of Social Work and the Department of Nursing.

Schedule: This position will work 80 hours per two weeks. The shift length will be 8-hours on the Day shift. Weekends: As Needed

As a Social Worker you will

Demonstrate patient-centered and customer focus by treating patients/customers as top priority to meet or exceed needs and expectations.

Contribute ideas and suggestions and participates in activities that improve systems, processes, and service for patients and customers.

Engage in monitoring and evaluating feedback and data from patients and customers to improve service and work unit's performance.

Contribute to the enhancement of group effectiveness by exchanging information, collaborating with other team members, and providing constructive feedback.

Listen attentively to patients and customers to ensure understanding and delivery of personalized service.

Help develop team cohesiveness by offering assistance to team members and aligns individual work efforts to meet work unit and organizational goals for service excellence.

Maintain a patient-centered, positive, and helpful orientation when working with individuals within and outside of the work unit.

  • Adjust to change in a positive and supportive manner.
  • Complete assignments on time, contributing extra effort to meet patients', customers', and colleagues' need
  • Adhere to patient confidentiality.
  • Conduct discussions of confidential information in appropriate settings to avoid unintentional disclosure.

Comply with all Fairview Range policies, procedures, department guidelines, and applicable laws in the conduct of routine activities and the solution of problems.

  • Perform a psychosocial assessment that reflects knowledge of illness and psychosocial issues
  • Coordinate referrals when indicated to the appropriate internal or community resources.

Function with supervisor to provide counseling to individuals, families, and groups to alleviate emotional distress, and facilitate adaptive behavior.

Collaborate with the multidisciplinary team in the development and ongoing evaluation of the patient's treatment plan depending on the setting.

Coordinate patient discharge planning process.

Comply with required standards for medical/legal responsibilities. Accountable to continuously improve and develop in the profession of social work and to support the continuous improvement of the department and institution.

Maintain professional standards and conduct.

Serve as a resource to nursing, medical, allied health, and administrative staff in the areas of consultation and education.

Required Qualifications

B. S./B. A. bachelor's level degree in social work from an accredited program

Licensed Social Work current in MN

Preferred Qualifications

  • Master of Arts master's degree in social work
  • 1 year experience

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
