Hospice Social Worker (MSW)

Key Responsibilities

The Hospice Social Worker assesses the psychosocial needs of patients and families while providing counseling and crisis intervention. They also serve as a liaison between the patient and community resources while participating in interdisciplinary care planning.

Requirements

Candidates must hold a master's degree in social work and have at least two years of documented supervised experience in health care. A valid driver's license and the ability to travel within the assigned service area are required.
