Key Responsibilities
Provide comprehensive primary care to a senior patient population within a value-based model. Manage complex chronic conditions while collaborating with a dedicated care team to ensure continuity of care.
Requirements
Must hold an MD or DO degree and be board certified or eligible in Family or Internal Medicine. Requires an active medical license, DEA registration, and at least two years of outpatient clinical experience.
