Responsibilities
Make a meaningful impact by evaluating, diagnosing, and treating patients across a variety of care settings.
You'll develop individualized treatment plans, establish functional goals, provide evidence-based interventions, educate patients and families, and collaborate with interdisciplinary teams to achieve exceptional outcomes.
Maintain accurate, timely documentation and billing, coordinate care with providers and community resources, participate in care conferences, and ensure compliance with regulatory and payer requirements.
Help shape a culture of excellence by supporting quality improvement initiatives, maintaining clinical competency, promoting patient safety, contributing to program development, and partnering with leadership to enhance the patient experience and operational performance.
Qualifications
Education:
Bachelor’s Degree required for applicants graduating from an accredited Physical Therapy Program prior to 2005.
Master’s Degree required for applicants graduating from an accredited Physical Therapy program between 2005 and 2019.
Clinical Doctorate (not PhD) required for applicants graduating from an accredited Physical Therapy Program between 2020 to present.
Experience
- UnityPoint at Home requires one (1) year of experience.
- Prior work as a Physical Therapist in practice area preferred.
- Recent Graduates will be considered.
License(s)/Certification(s)
- Licensed Physical Therapist in state(s) of practice required.
- IA Licensed Physical Therapist through the Iowa Department Health and Human Services (IADHHS)
- IL Licensed Physical Therapist through the Illinois Department & Professional Regulation (IDFPR)
- NE Licensed Physical Therapist through the Nebraska Department of Health and Human Services (NEDHHS)
- SD Licensed Physical Therapist through the South Dakota Board of Medical and Osteopathic Examiners (SDBMOE)
- WI Licensed Physical Therapist through the Wisconsin Department of Safety and Professional Services (WIDSPS)
- Basic Life Support (BCLS) required.
- IA Child Abuse Mandatory Reporter Training required.
- IL Child Abuse Mandatory Reporter Training required.
- IA Adult Dependent Abuse Mandatory Reporter Training required.
- IL Adult Dependent Abuse Mandatory Reporter Training required.
