Brighton/Suncrest is dedicated to providing exceptional care and service through a compassionate, supportive team that values communication, integrity, teamwork, and continuous growth.
Our Hospice Social Workers provide compassionate psychosocial support to patients, families, referring source and team members as they navigate the emotional, social, and practical challenges of serious illness and end-of-life care. You will serve as an advocate, resource, and trusted member of the interdisciplinary hospice team, helping patients and families receive the support they need throughout their hospice journey.
Qualifications
Master of Social Work degree that complies with CHAP and state of residence regulations.
Assess patients’ and families’ psychosocial, emotional, spiritual, and socioeconomic needs and incorporate findings into the individualized plan of care.
Counsel patients and families while providing emotional support, crisis intervention, and guidance as they navigate illness, loss, grief, and significant life transitions.
Educate patients and families about available community resources and connect them with appropriate programs, services, and support.
Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs, preferences, values, and goals are heard and addressed.
Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other members of the care team.
Maintain accurate, thorough, and timely documentation in accordance with hospice standards and regulatory requirements.
Key Responsibilities
Provide compassionate psychosocial support to patients and families while navigating the emotional and practical challenges of end-of-life care. Collaborate with the interdisciplinary team to develop and implement individualized care plans and advocate for patient needs.
