Behavioral Health Transitional Case Manager LCSW
Summary
The Southeast Permanente Medical Group (TSPMG) is one of Georgia’s largest independent, physician-owned, multi-specialty medical groups. We invite applications for a licensed Behavioral Health Transitional Case Manager LCSW to join our busy practice located in metropolitan Atlanta.

What You Can Expect
Provides direct behavioral health services within scope of practice, including post-hospitalization assessments, individual therapy, group therapy, and other clinically appropriate interventions
Maintains a rolling caseload of recently discharged members and conducts timely outreach, including multiple contact attempts as appropriate, to engage members and caregivers following discharge
Completes comprehensive assessments of members' psychosocial, behavioral health, environmental, financial, and functional needs; evaluates readiness for self-management and identifies factors that may affect treatment engagement outcomes
Utilizes clinical judgement to identify members at risk for relapse, deterioration, avoidable emergency department utilization, readmission, or disengagement from treatment, and escalates concerns to the appropriate care team members
Minimum Requirements
Licensed Clinical Social Worker
2 - 5 years inpatient psychiatric care
2 - 5 years discharge planning experience and/or transitional case management or complex case management experience working with behavioral health patients
