Home Health Physical Therapist (PT) PRN

Join our growing Home Health team and help patients regain independence, improve mobility, and achieve their goals in the comfort of their own homes. As a Home Health Physical Therapist, you'll develop individualized treatment plans, provide hands-on care, and build meaningful relationships with patients and families while enjoying the flexibility and autonomy that comes with community-based care.
What You'll Do
* Evaluate patients and develop personalized treatment plans.
* Improve mobility, strength, balance, and overall function.
* Educate patients and caregivers on exercises, safety, and disease management.
* Monitor progress and adjust treatment plans to maximize outcomes.
* Collaborate with physicians, nurses, and interdisciplinary team members.
* Complete timely and accurate clinical documentation.
Qualifications
* Graduate of an accredited Physical Therapy program.
* Current Physical Therapist license.
* CPR certification.
* Valid driver's license and auto insurance.
* Home Health experience preferred.
* HomeCare HomeBase experience preferred.
