Key Responsibilities
Provide comprehensive primary care to a senior patient population within a value-based care model. Manage complex chronic conditions while collaborating with a dedicated care team to ensure continuity of care.
Requirements
Requires an MD or DO degree with board certification or eligibility in Family or Internal Medicine. Candidates must hold an active, unrestricted medical license and have at least 2 years of outpatient experience.
