Qualifications

Key Responsibilities
The RN Case Manager will develop and implement individualized care plans while providing direct, compassionate support to hospice patients and their families. They are responsible for monitoring patient conditions, administering treatments, and collaborating with interdisciplinary teams to ensure high-quality end-of-life care.
Requirements
Candidates must hold a valid Registered Nurse license in good standing and possess a current CPR certification. A valid driver's license and reliable transportation are required to perform home visits.
