Key Responsibilities
Integrates healthcare for patients across the continuum of care by coordinating services and ensuring timely sequencing of care. Collaborates with multidisciplinary teams to remove barriers and monitor patient outcomes using clinical and utilization data.
Requirements
Requires a Bachelor's degree in Nursing and an unencumbered RN license. A minimum of two years of clinical experience is preferred.
