Job Description

Actively participates in quality assessment performance improvement teams and activities. 14. Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities. 15. Qualifications
POSITION QUALIFICATIONS 1. A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR 2. A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3). 3. The employee has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state. Previous home care/hospice is required. Experience in a hospice care preferred. 4. Demonstrates good verbal and written communication, and organization skills. 5. Possesses and maintains current CPR Certification (per state regulations) 6. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Key Responsibilities
The social worker assesses the psychosocial status of hospice patients and families to provide crisis intervention and counseling. They also coordinate with the interdisciplinary team to develop care plans and assist with end-of-life arrangements.
Requirements
Candidates must hold a bachelor's or master's degree in social work and possess a valid state license. Previous experience in home care or hospice is required, along with a valid driver's license and current CPR certification.
