As a Hospice Medical Social Worker, here’s a big-picture view of what you’ll do
* Conduct psychosocial assessments of patients and families and contribute to the development of the Interdisciplinary Group (IDG) plan of care
* Provide individual and family counseling related to the mental and emotional needs of patients and families
* Provide support and resources related to anticipatory and complicated grief, as appropriate
* Evaluate financial, environmental and other psychosocial concerns and connect patients and families with appropriate community resources
* Provide crisis intervention and emotional support to patients, families, caregivers and members of the hospice care team
* Participate in the development of the patient’s plan of care and regularly scheduled IDG meetings
* Help patients and families navigate end-of-life decisions, including advance directives, durable power of attorney, funeral arrangements and other financial, legal and healthcare matters
* Collaborate with physicians, clinical staff, facility staff and other members of the hospice team regarding significant social and emotional factors related to death and dying
* Participate in discharge planning when needed
* Provide ongoing psychosocial support and follow-up throughout the patient’s hospice experience
A few must-haves for Hospice Medical Social Workers
* Master’s degree in Social Work from a program accredited by the Council on Social Work Education
* Active Social Work license in Virginia and in good standing
* Minimum of 1 year of experience in healthcare
* Hospice experience preferred
* Knowledge of and commitment to the hospice philosophy of care
* Strong verbal and written communication and organizational skills
* Ability to work effectively as a member of an Interdisciplinary Group
* Compassionate, empathetic and able to communicate effectively with patients, families and caregivers
* Valid driver’s license and auto liability insurance if assignment includes driving your own vehicle
