About the Role

Join Hudson Headwaters Health Network as a Licensed Clinical Social Worker – Full-Time, $78K in United States. The Licensed Clinical Social Worker serves patients of the Network. He/she is a member of the patient-centered health care team at the assigned location(s), and coordinates care activities with other Providers.

Key Responsibilities

• May be assigned to offer “traditional” mental health services, which includes assessment, diagnosis, establishment of treatment goals and care plans, provision of mental health care and coordination

• Attentive to general medical concerns and treatments, and supports such interventions

• May be assigned to an “integrative” role, in which case he/she serves as a Behavioral Health Consultant (BHC) to a defined set of primary care providers

• Provides assessment & diagnosis

Requirements

• Must be certified to practice in New York State as a Licensed Clinical Social Worker, with a preference for NYS Department of Insurance “rating” to practice independently (the R rating).
