The Case Manager plans, organizes, and provides hospice care while directing the interdisciplinary team and family. They are responsible for developing patient care plans, ensuring compliance with documentation standards, and supervising hospice aides and licensed practical nurses.
Candidates must be a graduate of an accredited nursing program and hold a current Registered Nurse license and CPR certification. A minimum of two years of clinical, home health, or hospice experience is preferred, along with a valid driver's license and reliable transportation.
