Key Responsibilities

The RN Case Manager manages nursing care and coordinates individualized patient care plans across all age groups in a home health setting. They are responsible for assessing patient needs, documenting care, and collaborating with the interdisciplinary team to ensure positive outcomes and prevent avoidable rehospitalizations.

Requirements

Candidates must be a graduate of an accredited Registered Nurse program and hold a current license in Virginia or a compact state. A valid driver's license, automobile insurance, and a personal vehicle are required for making home visits.
