Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical South campus. This is a PRN Weekend shift opportunity working in the In-Patient Case Management Department at Baptist South campus.
Case Manager, Job Responsibilities
- Care coordination for hospitalized patients.
- High risk screenings and assessments.
- Coordinating post-acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
Experience Requirements
1 year Case Management experience
Education & Credential Requirements
- Associate's Degree Nursing (ADN)
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- FL Active licensed Registered Nurse (RN)

Key Responsibilities
The Case Manager is responsible for coordinating care for hospitalized patients and conducting high-risk screenings and assessments. They also manage the coordination of post-acute care services while collaborating with a multidisciplinary team.
Requirements
Candidates must have an Associate's Degree in Nursing and an active Florida Registered Nurse license. Additionally, at least one year of case management experience and Basic Life Support certification are required.
