Key Responsibilities

The Case Manager assesses, plans, and implements care services to meet patient health needs while ensuring cost-effective outcomes. They identify appropriate providers and facilities to support the overall care management process within the Emergency Department.

Requirements

Candidates must hold a nursing degree and be a licensed Registered Nurse in Massachusetts with 3-5 years of clinical experience. At least one year of case management experience is required, and CCM certification is preferred.
