Key Responsibilities
The RN Case Manager coordinates and facilitates quality, cost-effective care for ESRD and CKD patients by collaborating with healthcare teams and families. This role involves managing individualized care plans and supporting patients through complex healthcare transitions in a field-based capacity.
Requirements
Candidates must hold an active RN license in Ohio and Indiana with at least five years of clinical nursing experience. A minimum of an A.D.N degree is required, along with case management experience and proficiency in basic computer skills.
