Physician Assistant or Acute Care Nurse Practitioner - Hospital at Ho

Key Responsibilities

The provider will conduct clinical evaluations, determine medical eligibility, and manage the transition of patients from the hospital to the home setting. They will collaborate with interdisciplinary teams to ensure high-quality care, patient safety, and efficient throughput within the Hospital at Home program.

Requirements

Candidates must hold a Master's degree from an accredited nursing or Physician Assistant program and possess current Illinois licensure. A minimum of 5 years of acute care experience in fields such as Hospital, Emergency, or Critical Care medicine is preferred.
