Key Responsibilities

This social worker will provide assessment, intervention, and care coordination for pediatric patients at Helen DeVos Children's Hospital, addressing complex psychosocial needs including abuse, neglect, substance use, and mental health concerns. Responsibilities include collaborating with interdisciplinary teams, coordinating alternate site care, executing safe discharge planning, and maintaining current knowledge of community resources and regulations.

Requirements

A Master's Degree in Social Work is required for this position. Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning is preferred, along with holding the Master Social Work LTD license in the State of Michigan upon hire.
