The Medical Social Worker will provide psychosocial support to patients and families to help them navigate illness and recovery barriers. They will collaborate with a multidisciplinary team to develop individualized care plans and connect patients with necessary community resources.
Candidates must hold a Master's or doctoral degree from a CSWE-accredited school of social work and possess a current state license as a Master's Social Worker. A minimum of one year of experience and reliable transportation for daily travel are required.
