The Social Worker performs initial psychosocial assessments for potential PACE participants and coordinates with the interdisciplinary team to develop comprehensive care plans. They provide ongoing counseling and support to participants and families regarding aging, dementia, and end-of-life issues.
Candidates must hold a Master's degree from an accredited school of social work and possess a valid professional license. A minimum of one year of experience working with the frail or elderly population is required.
