The Home Health Medical Social Worker provides psychosocial support to patients and families to help them navigate illness and recovery. They collaborate with a multidisciplinary team to develop individualized care plans and connect patients with essential community resources.
Candidates must hold a Master's or doctoral degree from a CSWE-accredited school of social work and possess a current state license as an MSW, LMSW, or LCSW. A minimum of one year of licensed experience and reliable transportation for daily travel are required.
