VNA of Ohio is one of Northeast Ohio's most established home health and hospice agencies - 124 years strong, CHAP-accredited, and 4-star rated by CMS. We're building our PRN Hospice Nurse Practitioner team and offering competitive per-visit pay with the flexibility to work on your terms.
Pay Rates – Per Completed Visit
Per Visit - $90
Downtime (pre-approved) - $60/hour
"Completed" = visit done in the home + documentation finished within 24 hours + orders/signatures uploaded + meets QA standards.

What You'll Do
Conduct face-to-face hospice eligibility visits and complete required documentation in compliance with Medicare hospice regulations
Perform comprehensive assessments of hospice patients in their homes, including symptom management evaluation and goals-of-care discussions
Collaborate with the attending physician and interdisciplinary hospice team to develop and adjust individualized plans of care
Prescribe and manage medications focused on comfort, pain management, and symptom control
Provide advanced clinical guidance to hospice field staff including RNs, LPNs, and aides
Educate patients and families on disease progression, comfort-focused care, and end-of-life planning
Participate in interdisciplinary team meetings and case conferences as needed
Maintain accurate and timely documentation in the EMR within required timeframes
Ensure compliance with Medicare, Medicaid, and CHAP hospice standards

Requirements
Must-Have:
Active NP license (Ohio) + current BLS certification
Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Active DEA registration
Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
Minimum 2 years of advanced practice nursing experience
Familiarity with Medicare and Medicaid hospice regulations
Preferred / Nice-to-Have:
Prior hospice or palliative care NP experience
ACHPN certification or willingness to pursue
Experience conducting face-to-face hospice eligibility visits
Familiarity with HCHB or similar home health/hospice EMR
Strong goals-of-care conversation and family communication skills
