Job description

Join Heart to Heart Hospice as a Hospice Nurse Practitioner - PRN in Temple, Texas. You can Make A Difference in the lives of others! At Heart to Heart Hospice, we provide “Compassionate Care from Our Hearts to Yours.

Key Responsibilities

  • Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care.
  • Consult with and assists in the IDG’s development of patient/family plan of care to assure quality care is provided.
  • Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
  • Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.

Work Schedule

  • Per Diem position.

Work with Heart to Heart Hospice in Temple, Texas.

Requirements

Must be a graduate of a Baccalaureate School of Nursing with current RN and Advanced Practice Nurse (APN) licenses. Requires at least two years of APN experience in a clinical setting, preferably in hospice or home health.
