Key Responsibilities
The RN Case Manager completes ongoing physical and psycho-social assessments of patients and families while collaborating with the interdisciplinary group to develop and revise plans of care. Requirements
Candidates must hold an active, unrestricted Registered Nurse license in the State of Michigan and possess at least one year of relevant RN experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
