Key Responsibilities
Conduct psychosocial assessments and develop comprehensive care plans for hospice patients and their families. Collaborate with interdisciplinary teams to provide therapeutic support, advocacy, and end-of-life education while maintaining accurate clinical documentation.
Requirements
Requires a Bachelor's or Master's degree in Social Work from a CSWE-accredited institution. Must hold a valid LCSWA or LCSW license in North Carolina and/or LISW-CP in South Carolina.
