Key Responsibilities

Conduct psychosocial assessments and develop comprehensive care plans for hospice patients and their families. Collaborate with interdisciplinary teams to provide emotional support, resource referrals, and end-of-life care advocacy.

Requirements

Requires a Bachelor's or Master's degree in Social Work from an accredited institution. Must hold a valid LCSWA or LCSW license in North Carolina and/or South Carolina.
