Key Responsibilities
The Hospice Social Worker will conduct psychosocial assessments, develop care plans, and provide emotional support and counseling to patients and families. They are also responsible for coordinating community resources, facilitating bereavement programs, and collaborating with the interdisciplinary health team.
Requirements
Candidates must hold a Master's degree in Social Work and possess documented training in grief and loss counseling. A valid driver's license with a good driving record and reliable transportation is required for this role.
