Key Responsibilities
The Social Worker will conduct psychosocial assessments, develop care plans, and provide emotional support and counseling to patients and families. They are also responsible for coordinating community resources, facilitating bereavement programs, and providing education on hospice services.
Requirements
Candidates must hold a Master’s degree in Social Work and have documented training in grief and loss counseling. A valid driver’s license and reliable transportation are required for this role.
