Key Responsibilities
The Medical Social Worker provides compassionate, patient-centered support to hospice patients and their families to promote dignity and comfort. Responsibilities include counseling, resource navigation, advocacy, and collaborating with an interdisciplinary team to meet patient goals.
Requirements
Candidates must hold a Master of Social Work (MSW) degree and have at least one year of clinical geriatric experience. A valid California driver's license and a reliable vehicle are required for travel within the service area.
