Key Responsibilities
The Social Worker manages a caseload of hospice patients by performing psychosocial assessments and developing individualized plans of care. They also serve as a liaison to community agencies, facilitate discharges, and provide counseling to patients and families regarding grief and end-of-life arrangements.
Requirements
Candidates must hold a Bachelor's or Master's degree in Social Work and possess 1-3 years of experience in a healthcare setting. A valid driver's license and proof of automobile insurance are required, with a Clinical Social Worker license (LCSW) preferred.
