City/State: West Nyack, New York Grant Funded: No Department: ORTHO - NEOSM Work Shift: Day Work Days: MON-FRI Scheduled Hours: 8 AM-4:30 PM Scheduled Daily Hours: 7.5 HOURS Pay Range: $134,640.
Key Responsibilities
- will manage, monitor, and coordinate all aspects of the delivery of care for each patient to ensure coordination of care and education.
- will be responsible for ensuring that each patient receives the highest standards of care, and those standards will be measured in outcomes and program report cards.
- will monitor and evaluate patient satisfaction and implement programs to continually improve the quality of care and promote standardization of clinical pathways, along with improvements in overall satisfaction.
