Key Responsibilities

• will manage, monitor, and coordinate all aspects of the delivery of care for each patient to ensure coordination of care and education.

• will be responsible for ensuring that each patient receives the highest standards of care, and those standards will be measured in outcomes and program report cards.

• will monitor and evaluate patient satisfaction and implement programs to continually improve the quality of care and promote standardization of clinical pathways, along with improvements in overall satisfaction.
