Work Schedule

Key Responsibilities
The RN Case Manager will provide telephonic case management for medically complex members, conducting health assessments and creating individualized care plans. They will also coordinate care with partners, educate members on disease management, and monitor progress to ensure quality outcomes.
Requirements
Candidates must hold an active, unrestricted Registered Nurse license in California and possess at least two years of clinical nursing experience. Additionally, at least one year of case management experience and proficiency in Microsoft Office are required.
