Key Responsibilities
Provide primary care to a panel of seniors with complex chronic conditions in an outpatient clinic setting. Collaborate with a dedicated care team to ensure continuity of care and meet quality metrics such as patient satisfaction and HEDIS compliance.
Requirements
Must be a Board Certified or Board Eligible MD or DO in Family Medicine or Internal Medicine with an active, unrestricted medical license. Requires at least 2 years of experience in an outpatient environment and proficiency in basic office procedures.
