Key Responsibilities
Provide primary care to a panel of seniors with complex chronic diseases in an outpatient setting. Manage patient health through a value-based care model while collaborating with a dedicated care team.
Requirements
Must be a Board Certified or Board Eligible MD or DO in Family Medicine or Internal Medicine. Requires at least 2 years of experience in an outpatient environment and an active, unrestricted medical license.
