Occupational Therapist

Description

This is a remote, P/T contract position for an Occupational Therapist located in the Los Angeles County region. The Occupational Therapist will work directly with Senior homeowners, participating in its Older Adults Home Modification Program, to conduct in-home visual and written assessments to evaluate their mobility and functional abilities and any needs for adaptive equipment. Responsibilities include collaborating with NHS' Construction Project Managers to develop written home modifications plans and provide guidance for adaptive equipment use to ensure the best care outcomes. Additionally, the Occupational Therapist will provide oversight to guide scopes of work and approve modifications to ensure client-centered outcomes. The Occupational Therapist will also perform in-home follow-up assessments post-modifications to document patient progress to verify compliance with requirements and provide client education and training on adaptive equipment use, if needed.

Requirements

• Expertise in patient care for the aging preferred.
• Proficiency in identifying appropriate adaptive equipment and teaching clients how to use it effectively

Strong interpersonal and communication skills, with the ability to collaborate with patients, families, and interdisciplinary teams

• Bilingual in English and Spanish a plus.
• Completed degree in Occupational Therapy from an accredited institution

Certification and licensure: National Board for Certification in Occupational Therapy; California Board of Occupational Therapy

• Experience in working with diverse communities and understanding their unique challenges is highly desirable
• Strong organizational skills and the ability to manage multiple clients
• Proven ability to document patient progress effectively and adjust plans when necessary
• Have reliable transportation and a valid driver's license

Availability to perform in-home assessments all over Los Angeles County during normal agency business hours, Monday-Friday from 9am-6pm

Approximately 10+ hours/week, including at least one full day per week.
