Job Description
Heritage Hospice is excited to resume services in the Albuquerque community. Our dedicated team is back and ready to provide compassionate, high‑quality end‑of‑life care for patients and families across the region.
About the Role We are seeking a skilled and compassionate Nurse Practitioner to perform required Face‑to‑Face (F2F) encounters for hospice patients approaching benefit period recertification.
Key Responsibilities
- Conduct Face‑to‑Face encounters for patients entering their 3rd benefit period and beyond.
- Assess and document terminal prognosis, disease progression, functional decline, and symptom burden.
Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions.
Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices.
- Maintain timely, compliant documentation aligned with Medicare and state regulatory standards.
- Communicate findings effectively to physicians and clinical leadership.
Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings.
Qualifications
- Current and unrestricted Nurse Practitioner license in New Mexico.
- National board certification as NP (FNP, AGNP, or similar).
- Minimum 1-year clinical experience; hospice, palliative care, or geriatric experience strongly preferred.
- Solid understanding of hospice eligibility criteria and disease trajectories.
- Strong communication, interpersonal, and clinical assessment skills.
- Reliable transportation and willingness to travel across assigned territory.
