Key Responsibilities
Provides therapeutic interventions and psychosocial support to patients and families to ensure continuity of care within the hospital and community. Coordinates discharge planning and collaborates with multidisciplinary teams to address social determinants of health and patient needs.
Requirements
Requires a Master of Social Work (MSW) degree from an accredited school and current New Jersey LSW or LCSW licensure. Previous experience in a medical hospital social work setting is mandatory.
