Join Atlantic Health System as a Social Worker (LSW) Per Diem – Inpatient Case Management in Newton, New Jersey, United States. Atlantic Health System is seeking a per diem Social Worker (MSW) for Case Management at Newton Medical Center to work 8AM-4:30PM including one weekend every four weeks. Essential Functions/

The Social Worker is responsible for discharge planning within an acute care hospital setting. They also provide supportive and therapeutic counseling to patients and their families.
Requirements
Candidates must hold a Master of Social Work (MSW) degree and an active Licensed Social Worker (LSW) license in New Jersey. A minimum of one year of experience in an acute care medical setting or a relevant externship is required.
