Key Responsibilities
The RN Case Manager will conduct physical and psycho-social assessments to develop and revise patient care plans in collaboration with the interdisciplinary team. They are responsible for providing symptom management education and ensuring all necessary medical supplies and equipment are available for patient care.
Requirements
Candidates must hold an active, unrestricted Registered Nurse license in Michigan and possess at least one year of relevant RN experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
