Key Responsibilities

The RN Navigator coordinates the comprehensive rehab care continuum for patients, facilitating communication between patients, physicians, and the rehab team. They also track performance improvement measures and provide evidence-based education to staff and patients to optimize clinical outcomes.

Requirements

Candidates must hold an Associate’s Degree in Nursing and an active Kentucky Registered Nurse license. A minimum of two years of experience in rehab nursing and Basic Life Support certification are required.
