Key Responsibilities
The nurse navigator will oversee daily operations of the CMS GUIDE model, coordinating comprehensive care plans and providing support to patients and caregivers. They will also facilitate community engagement, manage program documentation, and collaborate with internal and external partners to ensure high-quality neuroscience care delivery.
Requirements
Candidates must hold a Bachelor's degree in Nursing with current licensure in KY/WV or a compact state. A minimum of 2-3 years of experience in aging, dementia care, or patient navigation is required, along with strong leadership and project management skills.
