Key Responsibilities
The Registered Nurse Field Case Manager will assess, plan, and implement individualized care management interventions for members in their homes or long-term care facilities. Requirements
Candidates must hold a current unrestricted Registered Nurse license in Washington D.C. and possess at least one year of experience in post-acute care and electronic medical records. A valid driver's license and the ability to work in a field-based capacity within the DC, MD, or VA area are required.
