Key Responsibilities

The Nurse Navigator coordinates patient care throughout the disease process and acts as a clinical liaison across medical services. They are responsible for patient education, symptom management, and facilitating the resolution of care gaps to prevent readmissions.

Requirements

Candidates must be a Registered Nurse with a current Illinois license and at least two years of clinical nursing experience. Strong interpersonal skills and the ability to work effectively within a multidisciplinary team are required.
