Key Responsibilities
The RN Case Manager manages patient transitions from acute or rehabilitation facilities to home, ensuring continuity of care through frequent outreach. They coordinate with multidisciplinary teams to reduce readmissions and emergency department visits for high-risk, medically complex patients.
Requirements
Candidates must hold a Delaware RN license or Compact Nursing License and a BSN degree. A minimum of 2-5 years of experience in acute or subacute care is required, along with BLS certification.
