Key Responsibilities

The Social Worker will plan, organize, and direct the Social Service Department to ensure residents receive appropriate psychosocial and medically related services. Responsibilities include conducting assessments, care planning, discharge planning, and facilitating interventions for residents, families, and physicians.

Requirements

Candidates must hold a 4-year degree and a valid Social Worker license in the State of Texas. At least 2 years of experience in a skilled nursing facility or hospital setting is preferred.
