Key Responsibilities
The Care Team Coordinator is responsible for overseeing a client care team and coordinating clinical care across various disciplines. They work with the Operations Manager to monitor care services and ensure the daily operations of the team meet customer needs.
Requirements
Candidates must have a bachelor's degree in an allied health field and a valid Pennsylvania Physical Therapist license. A minimum of 3 years of home care clinical experience is required, with management experience preferred.
